Instructions:

This Pre-Emergency Survey is strictly for emergency preparedness purposes to assist in making sure that students students are safe in the event of an emergency or disaster.

How the contact information you provide will be used:

If a disaster strikes, your instructor will publish a Post-Emergency Check-in Survey. If you do not fill out the Post-Emergency Check-in Survey, then your instructor may use any alternative contact information that you have provided in this survey to try to contact you to make sure you are safe.

In no way will your instructor use this alternative contact information to contact you about anything non-emergency related.
